PERSONAL LICENcE APPLICATIOn

Have your passport, driving license and NI number to hand and details of your living addresses over the last 5 years and the form usually takes about 10-15 minutes to complete. Our advice is to have this detail at hand before you start.

Do not wait until you pass your exam to start your application process.

Complete this form and upload your supporting documents immediately.

You will need to scan and upload these essential documents at the end of completing the form:

1 X copy of passport

1 X copy of driving license

1 X document showing your current address: bank statement, utility bill or council tax statement / bill

Scan and upload all 3 documents, but if you don’t have all 3, then you must have a minimum of any 2 out of the 3 to progress your licence application.

You must also scan and upload 2 X passport photographs - both of which must be signed by the applicant and one of which must be endorsed as a true likeness of you by a solicitor or notary, or person of standing in the community or any individual with a professional qualification AND MUST STATE “I certify that this is a true likeness of …..” - SCAN BOTH SIDES OF PHOTOS

You must also post the original photographs to our office, as we usually need the original copies for your application

Post to: APLH Personal Licence Services, 12 Beacon Lane, Heswall, Wirral, CH60 0DD

THE PROCESS AND HOW IT WORKS

All you need to do is complete this one online form and scan / upload and post the documents above to us - DONE!

You then concentrate on completing the course and passing the exam. We take care of the rest!

We are a registered agent and will complete a Disclosure and Barring Service (DBS) check on your behalf

We will complete the Disclosure of convictions and civil immigration penalties and declaration on your behalf

You took your exam through us so we automatically hold a copy of your certificate once you pass the exam

We pay all associated fees as these are included in your fee

We will complete the Personal Licence application on your behalf

We will then collate all the relevant application forms and supporting documents and compile your application

This will then be sent to you to electronically sign through DocuSign - you review and sign

We then submit your application to the relevant Local Authority
